Resumen OBJETIVO: Describir las características de las pacientes con cesárea por indicación de cesárea anterior. MATERIALES Y METODOS: Estudio cuantitativo, descriptivo, observacional y retrospectivo llevado a cabo en el Hospital Regional de Medicina Tropical Julio César Demarini Caro, Chanchamayo, Junín, Perú, entre enero de 2020 y junio de 2021. La información se recopiló de las historias clínicas completas y legibles de los sujetos de investigación y se reunió en una base de datos de Excel y se procesó en el programa SPSS versión 25. Se aplicaron estadística descriptiva, medidas de tendencia central y de frecuencias simples. RESULTADOS: La muestra estudiada fue de 200 mujeres que finalizaron el último embarazo por cesárea; la indicación fue, precisamente, cesárea previa. El 40% no tuvo otra indicación secundaria para la cesárea y 19% tuvo como indicación adicional el sufrimiento fetal agudo. Características obstétricas: multíparas (83.5%), con periodo intergenésico adecuado (62.5%), con atención prenatal adecuada (69%), y con pelvis ginecoide (84.5%). Complicaciones de las madres: anemia moderada y severa (64.5%), rotura uterina (10.5%) y atonía uterina (5%). Complicaciones perinatales: Apgar menor a 7 (15%), asfixia neonatal (11%) y síndrome de insuficiencia respiratoria (9%). CONCLUSIONES: Las indicaciones de las cesáreas previas fueron: sufrimiento fetal agudo, desproporción cefalopélvica, trastornos funiculares, trastornos hipertensivos, oligohidramnios severo y distocias de presentación. Las principales complicaciones materno-perinatales fueron: anemia moderada y severa, rotura uterina, atonía uterina, Apgar del recién nacido menor o igual a 7, asfixia neonatal, síndrome de insuficiencia respiratoria y recién nacido prematuro.


Abstract OBJECTIVE: To determine the main characteristics of patients with previous cesarean section. MATERIALS AND METHODS: Quantitative, descriptive, observational and retrospective study carried out at the Regional Hospital of Tropical Medicine Julio César Demarini Caro, Chanchamayo, Junín, Peru, between January 2020 and June 2021. The information was collected from the complete and legible medical records of the research subjects and was collected in an Excel database and processed in SPSS version 25. Descriptive statistics, measures of central tendency and simple frequencies were applied. RESULTS: The sample studied consisted of 200 women whose last gestation was terminated by cesarean section; the indication was, precisely, previous cesarean section. Forty percent had no other secondary indication for cesarean section and 19% had acute fetal distress as an additional indication. Obstetric characteristics: multiparous (83.5%), with adequate inter-gestational period (62.5%), with adequate prenatal care (69%), and with gynecoid pelvis (84.5%). Maternal complications: moderate and severe anemia (64.5%), uterine rupture (10.5%) and uterine atony (5%). Perinatal complications: Apgar less than 7 (15%), neonatal asphyxia (11%) and respiratory failure syndrome (9%). CONCLUSIONS: Indications for previous cesarean sections were acute fetal distress, cephalopelvic disproportion, funicular disorders, hypertensive disorders, severe oligohydramnios and presentation dystocias. The main maternal and perinatal complications were moderate and severe anemia, uterine rupture, uterine atony, newborn Apgar less than 7, neonatal asphyxia, respiratory distress syndrome and premature newborn.

ABSTRACT

Objetivo: Calcular taxa de parto vaginal e cesárea em pacientes com uma cesárea anterior e gestação a termo, bem como fatores associados à recorrência de cesaria- na. Métodos: Estudo caso-controle, por meio de dados de prontuário de gestantes a termo com uma cesárea prévia admitidas para parto na Maternidade Darcy Vargas do município de Joinville (SC), em 2019. Resultados: Foram analisadas 788 pacientes, das quais 331 (42,00%) tiveram parto normal (PN) e 457 (58,00%), cesárea (CS). O grupo PN foi composto por mulheres mais velhas (29; 28) e com mais gestações que o grupo CS, possuindo pelo menos um parto normal prévio (171; 57; p < 0,001). Como fator de risco para recorrência de cesárea, destacou-se a presença de colo desfavorável no momento do parto (47; 356; p < 0,001). Internação por trabalho de parto (284; 92; p < 0,001) e ruptura prematura de membranas (RUPREME) (33; 79; p = 0,030) estão entre os fatores de proteção para ocorrência de uma nova cesariana. Conclusão: A taxa de parto vaginal pós-cesariana (VBAC) foi de 42% e a de parto cesáreo foi de 58%, condi- zente com valores de referência mundiais. O fato de ter um ou mais partos normais anteriores e internar-se em trabalho de parto ou com RUPREME foi fator protetor contra a repetição da cesárea, enquanto o colo desfavorável no momento da inter- nação foi fator de risco. Há grande divergência na literatura, sendo necessários mais estudos para elaborar estratégias que auxiliem profissionais e pacientes a decidirem pela melhor via de parto após cesariana anterior.(AU)


Objective: To calculate the rate of vaginal and cesarean delivery in patients with pre- vious cesarean section and pregnancy to term, as well as factors associated with recur- rent abdominal delivery. Methods: Case-control study, by medical records of pregnant women to term with a previous cesarean section admitted for delivery at Maternity Dar- cy Vargas in the city of Joinville (SC) in 2019. Results: 788 patients, of which 331 (42,00%) had a normal delivery (PN) and 457 (58,00%) cesarean section (CS). The PN group was composed of older women (29;28), and who had more pregnancies than the CS group, having at least 1 previous vaginal birth (171; 57; p < 0,001). As a risk factor for cesarean recurrence, the presence of an unfavorable cervix at the time of delivery was highlighted (47; 356; p < 0,001). Hospitalization for labor (284; 92; p < 0,001) and premature rupture of membranes (33; 79; p = 0,030) are among the protective factors for the occurrence of a new cesarean section. Conclusion: The post-cesarean vaginal birth (VBAC) rate was 42% and the cesarean delivery rate was 58%, consistent with world reference values. The fact of having one or more previous normal bir- ths, hospitalization in labor or with premature rupture of fetal membranes were protective factors against the repetition of cesarean section, while the unfavorable cervix at the time of hospitalization was a risk factor. There is divergence in litera- ture, therefore more studies are needed to develop strategies that help professionals and patients to decide on the best way of delivery after a previous cesarean section.(AU)

ABSTRACT

Background: The incidence of cesarean section is steadily rising. Cesarean delivery has played a major role in lowering both maternal and perinatal morbidity and mortality rates.  There are various factors involved in the rise of rate of cesarean section like rising incidence of primary cesarean delivery, identification of high-risk pregnancy, wider use of repeat cesarean section, rising rates of induction of labor and failure of induction, no reassuring fetal status etc.Methods: A retrospective cross-sectional study was conducted. Data was collected from patient records of the hospital during 1st January 2018 to 30th June 2018. All patients (N= 602) who had delivered their baby by caesarean section were included in the study. Data were analyzed by help of SPSS version 21 after proper compilation.Results: Among all women who underwent cesarean section, majority were age group between 21 and 30 years (67.1%). Repeat cesarean section (48.5%), followed by fetal distress (18.9%), oligohydramnias (6.6%) and cephalopelvic disproportion (6.5%) were most common among all major indication of cesarean section. Intrapartum complication (2.0%) includes postpartum hemorrhage (1.2%), CS hysterectomy (0.5%), bladder injury (0.3%) and postpartum complication (2.3%) including UTI (0.8), wound infection (0.5%), sepsis (0.5%), lactation failure (0.5%) were major maternal complication of cesarean section.Conclusions: Reduction of number of primary cesarean section, successful VBAC, individualization of the indication and careful evaluation, following standardized guidelines can help to keep rate of cesarean section to the possible minimum level.

ABSTRACT

Severe life-threatening situations leading to a 搉ear-miss� event may arise unexpectedly in pregnancy. Delay in seeking help, delayed access to care, and poor quality of emergency obstetric services can lead to undesirable outcomes. Women meeting the WHO 搉ear-miss� criteria were assessed using a cross-sectional study design. These women were interviewed to evaluate the circumstances leading to a near-miss event. Reasons for delays in getting proper care were studied using the �delays� model.� Thirty-two women met the criteria for 搉ear miss� during the 15-month study period, with a maternal near-miss incidence ratio of 9.27/1000 live births. One or more delays were identified in 21 (65.6%) near-miss cases. Delayed access to care was the most important factor for delay. A review of near-miss cases can be used to improve and optimize the existing obstetric services.

ABSTRACT

Background: Emergency Peripartum Hysterectomy (EPH) is a lifesaving surgical procedure. The higher incidence of EPH in developing countries is because of the higher prevalence of risk factors of primary postpartum haemorrhage like, multiple pregnancy, grandmultiparity, cephalo-pelvic disproportion and prolonged obstructed labor/uterine rupture, previous caesarean section. The increasing incidence of the procedure in developed countries has been attributed to the increasing caesarean section rate which predisposes to placenta previa. Aims & Objective: To estimate the incidence, indications and complications associated with peripartum hysterectomy performed at a tertiary care hospital. Material and Methods: The retrospective observational analytical study was conducted at the Department of Obstetrics and Gynecology, Lady Goshen hospital, Mangalore from January 2012 to December 2012. Records of all patients who had undergone peripartum hysterectomy during the study period were explored for age, parity, booking status, indication and the type of operation performed. Maternal morbidity and mortality were also recorded. Results: During the study period there were 5497 deliveries, out of which 6 women had undergone an EPH. The incidence was 1.2/1000 deliveries. The main indication was uterine atony 4 (66.7%), followed by rupture uterus 1 (16.7%) and placenta previa with placenta accreta 1 (16.7%). Maternal complications included febrile illness (50%) and anaemia (33.3%). All women, 6 (100%) required blood transfusions, 2(33.3%) cases transferred to ICU. No maternal deaths occurred. Conclusion: The leading indications for emergency peripartum hysterectomy in our study are uterine atony, placenta previa with placenta accreta and uterine rupture. The higher incidence of emergency peripartum hysterectomy is because of the higher prevalence of risk factors like multiparity, previous caesarean section and cesarean section in the index pregnancy. Proper antenatal care, early identification of risk factors and timely intervention by an obstetrician are necessary in preventing this disastrous event.

OBJETIVO: Avaliar as repercussões da cicatriz uterina na dopplervelocimetria das artérias uterinas, entre 26 e 32 semanas, em gestantes primíparas com uma cesariana prévia, considerando quando esta foi realizada fora (cesárea eletiva) ou durante o trabalho de parto. MATERIAIS E MÉTODOS: Estudo prospectivo transversal em 45 gestantes, divididas em três grupos: 17 gestantes com cicatriz prévia resultante de cesariana eletiva (grupo A); 14 gestantes com uma cicatriz prévia oriunda de cesariana executada em trabalho de parto (grupo B); 14 gestantes cujo único parto anterior foi realizado por via vaginal (grupo C). A dopplervelocimetria das artérias uterinas foi realizada pela via abdominal. Foram calculados as médias, medianas e desvios-padrão (DP) para cada grupo em estudo. Em relação ao índice de pulsatilidade, a comparação dos grupos foi conduzida pelo teste não paramétrico de Kruskal-Wallis. RESULTADOS: Os valores médios do índice de pulsatilidade no grupo A variaram de 0,60 a 1,60 (média: 0,90; DP: 0,29), no grupo B, de 0,53 a 1,43 (média: 0,87; DP: 0,24), e no grupo C, de 0,65 a 1,65 (média: 1,01; DP: 0,37); p = 0,6329. CONCLUSÃO: Não houve repercussões da cicatriz de cesariana prévia na dopplervelocimetria das artérias uterinas avaliadas de 26 a 32 semanas de gestação.


OBJECTIVE: To evaluate the possible repercussions of the previous cesarean scar at uterine arteries Doppler velocimetry between the 26th and 32nd gestational weeks. MATERIALS AND METHODS: Prospective cross-sectional study including 45 women between 11 and 14 weeks of gestation, divided into three groups: 17 pregnant women with uterine scar resulting from a previous elective cesarean section (group A); 14 with uterine scar resulting from a previous cesarean section performed during labor (group B); and 14 pregnant women with a single previous vaginal delivery (group C). Uterine arteries Doppler velocimetry was performed with transabdominal approach. Means, medians and standard deviation (SD) were calculated for each group in the study. The groups comparison regarding pulsatility index was performed with the non-parametric Kruskal-Wallis test. RESULTS: In group A, the pulsatility index ranged from 0.60 to 1.60 (mean: 0.90; SD: 0.29), in group B, from 0.53 to 1.43 (mean: 0.87; SD: 0.24), and in group C, from 0.65 to 1.65 (mean: 1.01; SD: 0.37); with p = 0.6329. CONCLUSION: No repercussion of previous cesarean scar was observed at uterine arteries Doppler velocimetry performed in the period between the 26th and 32nd gestational weeks.

ABSTRACT

OBJECTIVE: The purpose of this study was to determine which factors influence the likelihood of successful trial of labor, to evaluate the risks of VBAC and to evaluate the criteria of dystocia and the safety of VBAC. METHODS: This retrospective study was peformed based on 61 pregnant women who succeeded VBAC, 37 VBAC failure women, 100 nulliparas and 100 multiparas. RESULTS: In clinical characteristics, the parity, the number of previous cesarean section, gestational age, estimated fetal body weight by ultrasonography, and neonatal body weight had notable differences between the success and the failure group. In obstetric and historical factors, cervical effacement, dilation, and Bishop score at admission, spontaneous labor and method of induction of labor had differences. Among these factors, estimated fetal body weight by ultrasonography, cervical effacement and augmentation of labor using oxytocin were the most prognostic factors affecting the success of VBAC. In VBAC group, the duration of active-phase, second phase and total duration of labor were 313.5, 36.4 and 350.2 minutes, which were significantly shorter than nulliparas, but longer than multiparas. CONCLUSION: The trial of labor after cesarean section will decrease repeat operation, if it is performed under exact understanding of successful prognostic factors and the unique time length of labor. It will contribute to increase advantages for mother and economic benefits.

ABSTRACT

Vesicouterine fistula, a fistula between bladder and uterus, is one of the rarest urogenital fistula. Most cases are associated with repeat Cesarean section and with vaginal birth after Cesarean section (VBAC). During Cesarean section, it is important to dissect and move the bladder away from uterus carefully, especially in case of adherent bladder to the anterior vaginal wall or lower segment wall and it is necessary to explain the possibility of vesicouterine fistula development in case of VBAC. Patients usually present with urinary incontinence in the early postoperative period or present months or years later with cyclic hematuria, amenorrhea, vaginal leakage of urine, urinary tract infection and secondary infertility. The diagnosis is made by cystoscopic visualization of the fistula orifice in the bladder and outlining the fistula tract on cystogram, hysterosalpingogram or transvaginal ultrasonogram. There have been varied approaches to the treatment of vesicouterine fistula with conservative therapy or surgical therapy. We experienced two cases of vesicouterine fistulas in women with previous Cesarean section, the one is delivered with repeat Cesarean section after trying VBAC, the other is delivered with vacuum assisted VBAC, and treated with surgical repair.

ABSTRACT

OBJECTIVE: To suggest the pregnancy outcome data according to the types of placenta previa in order to establish the optimal management of placenta previa. METHODS: A retrospective review of the clinical records of 179 women delivered with the diagnosis of placenta previa over 25 gestational weeks during the 6-year period from January 1, 1995 to December 31, 2000, at the Wonju Christian Hospital. We divided each groups into total, partial and marginal placenta previa in order to compare pregnancy outcomes. RESULTS: There were significant differences in the numbers of gravida, prior abortion, number of previous cesarean section between marginal and total placenta previa group. There were no significant differences in the gestational weeks at delivery, blood transfusion units, birth weight and placenta/birth weight ratio among each group. Statistically significant frequent hysterectomy in case of partial palcenta previa in comparison to marginal placenta previa was performed. Significant differences of the prior cesarean section (86.4% vs 3.8%) and placenta accreta (45.4% vs 37.5%) were noted between hysterectomy group and no hysterectomy group. CONCLUSION: Previous cesarean section history is strongly associated with cesarean hysterectomy. Women with placenta previa and history of previous cesarean section have more risk of placenta accreta. So, we should prepare sufficiently for cesarean hysterectomy in such cases.

ABSTRACT

Thrombocytopenia is hematologic disorder commonly occurs during pregnancy (5%) with different severity. It doesn't cause severe problem during pregnancy and after the delivery if it is not below certain level which will cause spontaneous bleeding. But in this case, patient was previous cesarean section status and platelet number didn't arise more than 20,000/microliter even after transfusion which will cause spontaneous bleeding. Vaginal delivery was done because transverse cesarean section was considered relatively safe although there was the risk of rupture of uterus, and was successful-both mother and baby is in good health condition. Here now we report this case because vaginal delivery of Immunologic thrombocytopenic purpuric woman with prior history of cesarean section has not reported.
